Appointment and Payment Policies
Your First Appointment
Please complete the Intake Forms provided to you electronically as soon as possible to help us understand your nutrition status so we can tailor your appointment to your individual needs and goals. Please bring your health insurance card and identification with you to your first appointment.
Provider Referrals
If you have a chronic disease such as diabetes, kidney disease, heart disease, or obesity, you may need a provider referral for Medical Nutrition Therapy. We can provide you with the proper forms to give to your provider and they can fax the completed referral to our office prior to your first visit.
The Counseling Process
Once your Intake paperwork is complete, you will meet with the dietitian for a comprehensive nutrition assessment. Together, you will identify goals and develop a plan of action to meet your goals. We generally recommend Follow-up visits at weeks 3 and 6 and every 12 weeks thereafter, depending on the service being provided. Follow-up visits allow us to make adjustments to your plan of action so you can stay on track to achieving your goals.
Paying for Nutrition Services
Our fees are billed on a per session basis and are due at the time of service. Most health insurance plans cover some form of Nutrition Counseling Services and/or Medical Nutrition Therapy. We are currently contracted with Medicare, Presbyterian Health Plan, Blue Cross Blue Shield, United Healthcare, and Cigna. As a professional courtesy, we will assist you in verifying your insurance coverage to determine whether our services are covered and, if appropriate, we will bill your insurance company directly for our fees. Upon request, we can also provide you with a Superbill for you to submit to your insurance company for reimbursement. Please know you are responsible for payment of any deductibles, co-pays, or co-insurance not covered by your insurance company at the time of service. Advanced Nutrition Services accepts cash, checks, credit and debit cards, Flex Spending Accounts (FSA) and Health Savings Accounts (HSA) with a Letter of Medical Necessity (LMN) from your healthcare provider. There will be a $25 charge for checks that are returned unpaid.
Sliding Fee Discount
If you are uninsured, underinsured or if your financial circumstances do not allow you to pay our full-fee, you may be eligible for our Sliding Fee Discount program which allows for payment that is less than our full-fee schedule. Appointments
Appointments can be made online, by phone, or email. You will receive an appointment confirmation by email as well as Intake Forms to complete before your first visit.
Cancellations
If you are unable to attend a scheduled appointment, please notify us as soon as possible. There will be no fee if you cancel or reschedule your appointment at least 24 hours prior your appointment time. If you miss your appointment or cancel less than 24 hours prior to your appointment, please understand you will be charged a $25 missed appointment fee.
